A Return and Refund policy usually consists of:

 

  • Premium: The amount you pay for your insurance coverage. 
  • Deductible: The amount you may need to pay for covered services before your plan begins paying its share. 
  • Copayment: A fixed amount you may pay for a covered healthcare service or prescription. 
  • Coinsurance: The percentage of a covered cost you may pay after meeting your deductible. 
  • Provider Network: The doctors, hospitals, pharmacies, and other healthcare providers that participate with a health plan. 
  • Out-of-Pocket Maximum: The most you may have to pay for covered in-network services during a plan year before the health plan pays 100% of covered benefits, subject to the plan's terms.
